Preventive care is fully covered at no cost to you. Virtual visits and behavioral health appointments are available 24/7 for $0. And if you participate in our Wellness Incentive Program, your premium goes down even further.

Medical — United Healthcare Choice Plus
Comprehensive in-network coverage through Presbyterian, UNM, Optum, and Lovelace. Primary care visits start at $20, specialists at $40, and emergency room visits at $200. Routine lab work and X-rays at freestanding facilities are fully covered. Out-of-pocket maximum of $3,500 individual / $7,000 family.

See a doctor or therapist from home, 24 hours a day, 7 days a week — at no cost to you. Available for common illnesses, mental health, and behavioral health support.

Dental — MetLife
Two plan options — Base and Buy-Up — both covering preventive, basic, and major dental services. Orthodontia included. Choose from Base ($1,500 annual max) or Buy-Up ($2,000 annual max, adult orthodontia included).

Annual exams, lenses, and frames covered. Includes Costco, Sam’s Club, and Walmart. Lens upgrades like anti-reflective coating and photochromic lenses covered in full in-network.

Prescriptions
Retail prescriptions start at $10 for generics, with mail order available at a reduced rate for a 90-day supply. Certain preventive medications are covered at 100%. Check the formulary to find the lowest cost option for your medications.
